Yes, it has to be prescribed. There are numerous forms of B12, the most effective being methylcobalamin. B12 is included in our $299 per month package, which includes the following:

*Everything included in the $199 per month package plus the following
*Testosterone support supplement, which includes DHEA
*Fish Oils

*Injectable B12 (methylcobalamin)

1. Is all of the above in a injection or are they all in pill form aside from the B12?
2. Is the DHEA a sustained release? I am currently taking 50mg SR daily on my own.
3. How do we determine DHEA dosage as well as the vitamins?
4. are all of these products then supplied as part of the additional $100?
5. Since the Injectable B12 is prescription I would guess the $15 prescription applies, correct? What about the others since they are OTC?

That's correct, all oral except the B12

It is listed in the ingridients as DHEA, not SR. However, it's taken twice per day. The support formula has a morning and nighttime version, both contain DHEA.
3. How do we determine DHEA dosage as well as the vitamins?
Simply follow the directions on the bottle. The amounts contained are listed on the bottle. If the dr. directs you to take more or less that's up to you and the dr.

Yes

Yes, it would apply to the B12 but it would not for the other items.
