Newbie 29 yr old male with low T. Would you ask for treatment with these numbers?

I am new to the forum and testosterone issues in general. I hope someone will be kind enough to offer their opinion.
Me: 29, 165lbs (have put on 10 lbs over the last year despite no changes in diet and exercise), healthy eater, hardly any alcohol, no tobacco, no drugs.
History: no trauma to testicles, never used steroids, took accutane as an 18 year old, no history of depression and never used SSRIs. Possibly caught dengue in Sudan in 2011 although not formally diagnosed. Seen numerous tropical disease specialists and hat a battery of blood tests for diseases and nasty parasites. Nothing came up. I don't over train. No blows to the head.
My story: Over the last three years I've had bouts of chronic fatigue, mild depression and libido has plummeted. Generally can't stay up past 9:30 PM. Get chills in the evenings sometimes. Often get aches and pains early evening. Basically, I feel wiped out a lot of the time. The past 6 months has been particularly bad, hence a visit to the doctor.
The doctor initially wanted to check my thyroid, I requested test to be added. Just a gut feeling I have had for a while.

Endoc Notes:

It seems as though it is case closed as far as the endocrinologist is concerned. The problem is that I have been symptomatic for some time, and remain so.
Any ideas? Has anyone my age and background had similar numbers? I would greatly appreciate some feedback.
